When I tried to submit a copyright removal webform, it always says: Unable to submit removal request

Question

I tried to submit a copyright removal form to take down a copyright video of mine (not from Youtube), I completed all the necessary information that required, but when I clicked on "Submit", it said "unable to submit removal request", without telling what the problem is, does anyone know what could be the issue?
The previous version of the DMCA takedown form worked very well, why the latest version don't work anymore?
BTW, I also tried to send the information required via email to Copyright@Youtube.com, all I got is some auto-reply which told me to go back to the webform, which is just rediculous.

Quick update - this has been resolved and you should be able to use the webform without any issue :)
